Project Manager - Family Resource Center Tracking Code 1064928_RR00037822 Job Description Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes nine primary care sites, 33 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs. Position Summary:We have an exciting opportunity to join our team as a Project Manager.The Project Manager of SDOH Projects will identify best practices to engage patients and connect them to needed services. He/she will coordinate and track the social determinants of health screening and related close looped referrals for family health center patients and will use internal data and best practices in the field to identify ways to streamline the screening and referral processes. Job Responsibilities: + Work with research department and data team to evaluate the effectiveness of SDOH screening projects. + Work in collaboration with MCIT and Data team to develop reports that can more effectively track outcome of screenings with a focus on which patients are most likely to be assisted + Develop and implement training for health center staff on how to implement SDOH screenings + Meet with referred clients to assess and refer for needed services + Identify ways to increase efficiencies in assessment and referral process to that SDOH screening can be brought to scale. + Work with community partners and internal programs to ensure that referral network is able to meet needs that patients are being screened for. + Collaborate with research staff, site staff, and PIs on SDOH research grants to ensure that everyone is communicating effectively about project implementation + Work with supervisor to assign patients that screen positive and to ensure that follow up is provided and communicated in a timely manner. + Assist in training of interns, AmeriCorps members, and staff in how to follow up with patients that have screened positive for different needs. + Collaborate with IT and key program staff to use technology to effectively screen and provide closed loop referrals. Additional Position Specific Responsibilities:At least two years of experience in providing case management or related services and one year of experience in project management or coordination.
